Thank you for taking the time to submit a nomination for the Smiths Falls Recreation Hall of Fame.

Once complete, your application will be provided to the Hall of Fame Committee for consideration. Incomplete applications may not be considered.

Criteria details are as follows:

1. Nominees may have been an athlete, organizer, institution or team
2. Nominees may be either living or deceased and must have been a resident of Smiths Falls for a minimum of 10 years
3. Nominees may have been involved in sports, recreational and cultural activities
4. Persons nominating potential inductees should bear in mind that a maximum of 4 individuals per year will be honoured
5. All previous nominations must be reviewed annually prior to consideration of current nominations or suggestions from committee member
